Carta now supports customizable form fields in agreement templates, letting you collect tailored signer info during execution—streamlining workflows and data collection.

With custom form fields for LLC documents, you can embed data collection directly into the agreement signing workflow, reducing excessive back-and-forth in the signing process.

Now, you can tailor agreement templates to gather specific information—tax IDs, acknowledgements, or other declarations—in addition to the signature itself. This streamlines execution and keeps documents complete, accurate, and audit-ready.

How it works:

Templates can now contain custom-form fields uniquely tailored to various signers—whether it's the admin, investor, spouse, or others involved. 

Each field can be set as required or optional, with customized prompts clarifying exactly what signers need to provide. 

Supported field types include:

  • Text fields

  • Dates

  • Phone numbers

  • Checkboxes

  • Single-choice selections

  • SSN/EIN number fields for IRS forms
